Título: INFILTRATIVE FACIAL LIPOMATOSIS: A RARE CASE REPORT

Resumo: A 56-year-old female presented with lower lip enlargement, unilateral facial asymmetry, and limited mouth opening since childhood, with slow progression, consistent with a congenital condition. Imaging revealed condylar remodeling with associated osseous enlargement and diffuse fatty infiltration of adjacent soft tissues, including partial replacement of the masticatory musculature. The main diagnostic considerations included lipoma, diffuse lipomatosis, and hamartomatous lesions. Histopathological examination showed non-encapsulated mature adipose tissue with an infiltrative pattern and no cellular atypia. Immunohistochemistry (SMA, CD34, HHF-35, S100) supported lesion characterization but was not independently diagnostic. The diagnosis of congenital infiltrative facial lipomatosis was established based on the integration of clinical history, imaging findings, and histopathological features. Due to the extensive infiltration, conservative management with regular follow-up was adopted. This case highlights the importance of clinicopathological and imaging correlation in distinguishing infiltrative lipomatous lesions from other benign adipocytic tumors.
