Resumo: A 43-year-old female non-smoker and non-drinker with a history of partial glossectomy performed one year earlier for a well-differentiated keratinizing squamous cell carcinoma with clear margins and no angiolymphatic or perineural invasion presented with a heterogeneous erythroleukoplakia in the same region. Biopsy revealed hyperkeratosis with intracellular edema and mild subepithelial mononuclear inflammation. Although a repeat biopsy was proposed, the patient declined, and strict clinical follow-up was recommended. Four years after the initial intervention, the patient developed an indurated ulcerative lesion arising within the erythroleukoplakia. A new biopsy confirmed recurrent squamous cell carcinoma with keratin pearl formation, nuclear pleomorphism, and skeletal muscle invasion. The patient was referred for oncologic management and initiated preventive dental care in anticipation of possible chemoradiotherapy. This case highlights the diagnostic challenges of recurrent oral squamous cell carcinoma when interim biopsies suggest benign conditions, reinforcing the importance of strict long-term clinicopathologic follow-up.
