Resumo: Oral lymphoepithelial cyst (OLEC) is a rare benign lesion, accounting for less than 1% of oral cavity lesions, typically presenting as an asymptomatic submucosal nodule, with predilection for the floor of the mouth and tongue. A 58-year-old male patient presented with an asymptomatic nodule in the floor of the mouth, approximately 1.0 cm in diameter, well-circumscribed, yellowish, and with a smooth surface. The initial diagnostic hypothesis was lipoma. An excisional biopsy was performed, and the specimen was submitted for histopathological analysis. On macroscopic examination, sectioning revealed yellowish content, raising suspicion of a cystic lesion. Histopathological analysis showed a cystic cavity lined by parakeratinized stratified squamous epithelium, with keratin in the lumen and a fibrous capsule containing dense lymphoid infiltrate with germinal centers, confirming the diagnosis of OLEC. Surgical excision was curative, with no recurrence after 4 months. This case highlights the importance of clinicopathological correlation for accurate diagnosis.
