Título: SERUM sPD-L1 AND PD-L1 ISOFORM 4 EXPRESSION IN HEAD AND NECK SQUAMOUS CELL CARCINOMA

Resumo: Objective: To investigate the correlation between serum levels of soluble PD-L1 (sPD-L1) and the gene expression of PD-L1 isoform 4 (secPD-L1) in tumor tissue, exploring their association with clinico-pathological parameters and survival in patients with head and neck squamous cell carcinoma. Study design: This was a prospective analytical observational study (n = 12). Serum sPD-L1 levels were quantified using enzyme-linked immunosorbent assay (ELISA), and isoform 4 expression in fresh tissue was determined by RT-qPCR. Statistical analysis included Mann-Whitney tests, Spearman's correlation, and the Kaplan-Meier method, with significance set at p ≤ 0.05. Results: Elevated sPD-L1 levels were associated with advanced clinical staging (III/IV) (p = 0.042) and larger tumors (T3/T4) (p = 0.053). Conversely, high expression of isoform 4 showed a trend toward a better prognosis, with a higher mean survival (27 months) compared to low expression (11.5 months) (p = 0.065). No correlation was found between sPD-L1 levels and isoform 4 expression (p = 0.897), indicating independent behavior. Conclusion: Serum sPD-L1 acts as a marker of aggressiveness and tumor burden, whereas isoform 4 suggests a favorable prognostic role, highlighting the distinct biological functions of these molecules within the tumor microenvironment.
