Título: HEAD AND NECK SQUAMOUS CELL CARCINOMA IN NON-SMOKING/NON-DRINKING PATIENTS: A SYSTEMATIC REVIEW AND META-ANALYSIS

Resumo: Objective: To compare the clinicopathological characteristics and survival outcomes of non-smoking/non-drinking (NSND) and smoking/drinking (SD) patients with head and neck squamous cell carcinoma (HNSCC). Study Design: A systematic review and meta-analysis were conducted following a search of major databases on September 22, 2025. Risk of bias was assessed using the Joanna Briggs Institute tools, and meta-analyses were performed in Review Manager (RevMan). Results: NSND patients were more frequently female and exhibited tumors predominantly in the oral cavity, followed by the oropharynx and larynx. Meta-analysis demonstrated that NSND patients presented with earlier tumor and clinical stage, and reduced nodal involvement and lymphovascular invasion compared with SD patients. NSND patients also demonstrated significantly improved overall survival and disease-specific survival, whereas disease-free survival did not differ between groups. Conclusions: NSND appears to represent a distinct clinicopathological subgroup with more favorable tumor characteristics and prognosis. Funding: FAPESP-2023/14770-4 and 2024/05212-0
