Resumo: Metastatic hepatocellular carcinoma (HCC) to the oral cavity is exceedingly rare. We report a case of a 53-year-old male with chronic hepatitis B presenting with advanced HCC, characterized by rapid hepatic progression, IVC infiltration, and pulmonary metastases. Serum alpha-fetoprotein (AFP) rose significantly from 14 ng/mL to 227.5 ng/mL. While receiving palliative treatment with intravenous (IV) Atezolizumab (1200 mg) and Bevacizumab (15 mg/kg) every three weeks, the patient developed a 5 x 4 cm fungating, bleeding mandibular mass. An incisional biopsy was performed on the buccal side of the mass. Immunopositivity for hepatocyte-specific antigen (HSA) and a canalicular pattern of CD10 confirmed a high-grade metastatic carcinoma of hepatobiliary origin. Following uneventful surgical debulking and COE-PAK placement, the patient died six days later. Oral HCC metastasis signifies terminal stage of disease progression, requiring prompt diagnosis and prioritized palliative care intervention.
