Resumo: OBJECTIVE: To determine the demographic, clinical, radiographic, histopathological, and genetic characteristics of incisor-molar root malformation (IMRM) reported worldwide in the literature.

STUDY DESIGN: A scoping review was conducted across 11 databases, with no language or date restrictions. Of 3.048 identified studies, 39 met the inclusion criteria. Case series and case reports were selected.

RESULTS: A total of 159 patients were analyzed, with median age of 9 years and a slight male predominance. Medical history was relevant in 92% of cases. Most affected teeth were permanent (79.3%), predominantly first molars, with 39 different patterns. Clinically, enamel defects in incisors, pain and mobility in molars were observed. Radiographically, incomplete root development and narrowed pulp chambers were the most prominent findings. Histopathological analysis revealed alterations in dentin (100%), pulp (86.7%), and cementum (80%), with the characteristic presence of a mineralized cervical diaphragm within the pulp chamber. At the genetic level, TCTEX1D2 was the only gene directly associated, while NFIC emerged as the main candidate gene.

CONCLUSION: IMRM is a rare anomaly with distinctive clinical-radiographic and histopathological features, characterized by alterations in dentin, pulp and cementum, with the presence of a mineralized cervical diaphragm. Further studies are required to elucidate its etiopathogenesis.
