Resumo: Objective: To evaluate the preoperative neutrophil-to-lymphocyte ratio (NLR) and tumor-infiltrating lymphocyte (TIL) density in patients with mucoepidermoid carcinoma (MEC). Study design: Retrospective cross-sectional study using clinical and pathological data from medical records. Preoperative NLR was obtained from blood counts, and archived histological slides were analyzed to assess TIL density following TIL Working Group criteria. Associations were tested using the Pearson chi-square test. Results: Thirty patients were included (mean age 41.6 years; 63.3% female). Low-grade tumors predominated (56.7%), with the parotid gland as the most frequent site (46.7%). Low NLR was observed in 62.5% of cases, mostly in low-grade tumors (70%), whereas high NLR was associated with intermediate/high-grade tumors (83.3%). NLR was significantly lower in patients without perineural invasion (p=0.04) and in low-grade tumors (p=0.03). Low TIL density (29.6%) was more frequent in higher-grade tumors. Intermediate TIL density was linked to poorer outcomes, including all metastatic cases. High TIL density (26%) was associated with low-grade tumors (71.4%) and absence of perineural invasion (p=0.02). Conclusion: Higher NLR was associated with more aggressive MEC, whereas higher TIL density correlates with less aggressive features.
