Título: MALIGNANT TRANSFORMATION IN ORAL LICHEN PLANUS AND ORAL LICHENOID LESIONS: A RETROSPECTIVE STUDY

Resumo: Objective: The study aimed to determine the malignant transformation (MT) rate of oral lichen planus (OLP) and oral lichenoid lesions (OLL). It also evaluated the prevalence, socio-demographic, clinical and histopathological features associated with these lesions.

Study design: A retrospective study was conducted by retrieving pathologic reports of OLP, OLL and oral epithelial dysplasia (OED) with a provisional diagnosis of OLP/OLL from the year 2008-2018. Clinical records were reviewed for relevant clinical features and histopathological slides were reassessed to evaluate the histopathological features. Cases were subsequently reclassified based on modified clinical and histopathological criteria and MT was assessed from clinical follow-up.

Results: The prevalence of OLP/OLL was 5.7%. Most of the cases were OLL, followed by OLP and OLP/OLL with dysplasia. The overall MT rate for OLP was 1.89%. No MT was observed in OLP or OLL alone, while OLP/OLL with dysplasia showed a higher MT rate of 12.5%. The most common dysplastic features seen were premature keratinization in single cells and generalized premature keratinization.

Conclusion: OLP/OLL with dysplasia showed the highest MT rate. Careful monitoring and regular follow-up are essential, particularly those with dysplastic changes.
