Título: INTRAOSSEOUS PLASMACYTIC PROLIFERATION IN THE JAWS AS THE FIRST MANIFESTATION OF MULTIPLE MYELOMA: A CASE REPORT

Resumo: A 48-year-old female patient with leucoderma was seen at a private clinic for a routine examination and presented with intraosseous radiolucent lesions in the maxilla and mandible, with an indeterminate duration of evolution. An incisional biopsy was performed in the maxillary tuberosity and at the base of the mandible. Histopathological examination revealed thin fragments of trabecular bone, with wide medullary spaces filled with trilineage hematopoietic tissue, including erythroid, myeloid, and megakaryocytic series at different stages of maturation, interspersed with medullary adipose tissue. Immunohistochemical analysis showed positivity for kappa and lambda light chains, as well as expression of the plasma cell marker CD138. A high Ki-67 proliferation index (20-30%) was observed. Based on the histopathological and immunohistochemical findings, a diagnosis of plasmacytic proliferation was established. After complementary tests, the diagnosis of multiple myeloma was confirmed, with involvement of the skull, upper limbs, and thorax. The patient was referred for oncological care.
