Título: PRIMARY SYPHILIS ON THE LINGUAL APEX WITH NON-REACTIVE VDRL: A CASE REPORT

Resumo: Oral primary syphilis classically manifests as a hard chancre, frequently as a painless ulceration. A 58-year-old male patient presented complaining of a painless lesion on the lingual apex with a 10-day evolution, reporting sexual contact with an individual suspected of having syphilis. The VDRL test was non-reactive, a finding justified by the initial immunological window of the infection. Given the strong clinical suspicion, a biopsy of the lesion was performed. The histopathological report confirmed the definitive diagnosis of primary syphilis. The patient was referred for standard systemic treatment with benzathine penicillin G. Clinical follow-up after 15 days showed complete remission of the lesion and excellent healing of the biopsied surgical area. This case highlights the importance of biopsy for the assertive diagnosis of suspected syphilis lesions when initial non-treponemal tests have not yet become positive.
