Título: FRACTAL DIMENSION AND PIXEL INTENSITY ANALYSIS OF ORAL LEUKOPLAKIA IN CLINICAL IMAGES

Resumo: Objective: Fractal analysis quantifies the geometric complexity through fractal dimension (FD). This study evaluated FD and pixel intensity (mean and standard deviation) in clinical images of oral leukoplakia (OL), comparing homogeneous (H) and non-homogeneous (NH) lesions. Study design: Fifty clinical photographs were obtained and classified as H or NH by 10 evaluators with weighted voting based on expertise. Regions of interest (100×100 pixels) were selected from biopsy-chosen representative lesional areas and analyzed using ImageJ/Fiji. Pixel intensity was assessed in 8-bit grayscale images (mean [M] and standard deviation [SD]), and FD was calculated using the box-counting method after image binarization and skeletonization. Results: Final classification included 24 H and 26 NH. H lesions showed higher M values than NH (177±25 vs. 148±25; p=0.0006, Mann-Whitney test), while SD was higher in NH lesions (21±5.4 vs. 15±5.7; p=0.0005, unpaired t-test). No significant difference in FD was observed (p=0.50), although it was observed that the skeletonized linear structure matched white and red components in different images. Conclusion: Homogeneous OL lesions exhibit higher pixel intensity and lower variability than non-homogeneous lesions. FD did not discriminate between groups, likely due to the coexistence of distinct structural patterns, which were skeletonized differently and limited its interpretability.
