Título: Proteogenomic dissection of benign salivary gland tumors uncovers key oncogenic pathways and diagnostic signatures

Resumo: Background: Benign salivary gland tumors remain poorly characterized at the molecular level despite their clinical heterogeneity and potential for malignant transformation. This study applies an integrative proteogenomic approach to elucidate tumorigenic mechanisms and identify potential diagnostic biomarkers. Study design: Whole-exome sequencing and label-free quantitative proteomics were performed on pleomorphic adenoma, myoepithelioma, and basal cell adenoma, alongside normal salivary gland controls. Bioinformatic integration, pathway enrichment, and validation analyses, including malignant counterparts, were conducted. Results: All benign tumors maintained a predominantly diploid profile, resembling normal salivary tissue. Genomic analysis revealed a high mutational burden with recurrent alterations in oncogenes and tumor suppressors, notably converging on PI3K/AKT, TP53, and WNT signaling pathways. Proteomic profiling identified tumor-specific loss of salivary function proteins and enrichment in extracellular matrix (ECM) remodeling and metabolic reprogramming. Integrative analysis demonstrated concordant activation of WNT/β-catenin signaling (CTNNB1) and dysregulation of MTOR signaling, marked by MTOR overexpression and 4EBP1 loss. Distinct proteomic signatures, including ECM-related proteins (e.g., COL2A1, MATN3), emerged as candidate biomarkers for tumor subclassification. Conclusion: Benign salivary gland tumors exhibit complex molecular alterations involving canonical oncogenic pathways and metabolic shifts. Integrative proteogenomic provides novel insights into tumor biology and identifies potential diagnostic and therapeutic targets. FAPESP Funding: 22/07410-9; 24/05212-0.
