Resumo: A 41-year-old male patient presented with a mild complaint of maxillary discomfort for one year, partially relieved by anti-inflammatory medication. The patient was undergoing dental implant treatment planning, with a bone graft procedure scheduled based on computed tomography findings. Clinically, an erythematous vestibular sulcus lesion was associated with right maxillary expansion, extraoral swelling, and subtle ipsilateral ocular proptosis. Cone-beam computed tomography demonstrated an ill-defined, expansile osteolytic lesion involving the right maxilla. An incisional biopsy was performed, and the histopathological analysis demonstrated malignant epithelial cells forming duct-like islands, with ovoid to plasmacytoid morphology, hyperchromatic nuclei, and positivity for cytokeratin 7, leading to a diagnosis of adenocarcinoma. The patient was referred to an oncology center for surgical management and multidisciplinary care. This case reinforces the critical need for comprehensive diagnostic assessment prior to implant-related procedures, aiming to prevent delayed diagnosis and potentially harmful interventions in rare maxillary neoplasms.
