Título: Acinic Cell Carcinoma: An Unusual Presentation in Minor Salivary Glands

Resumo: A 69-year-old female presented with a two-year history of an oral lesion. She was a former smoker, non-alcohol user, and had no comorbidities. Clinical examination revealed a 2-3 cm submucosal, sessile nodule in the retromolar trigone, with purplish coloration, telangiectasias, smooth surface, and well-defined margins. No extraoral alterations were observed. Incisional biopsy revealed a malignant glandular neoplasm composed of atypical acinar cells with basophilic granular cytoplasm, small central nuclei, cytoplasmic vacuolization, and focal hyalinization. Immunohistochemistry showed positivity for CK8. Periodic acid-Schiff (PAS) staining confirmed the absence of mucin. A final diagnosis of acinic cell carcinoma (ACC) was established. The patient was referred to an oncology service for treatment. This case highlights the importance of considering malignant salivary gland neoplasms in the differential diagnosis of lesions in the retromolar region. Although rare, ACC may arise in minor salivary glands, reinforcing the need for careful diagnostic evaluation and histopathological confirmation.
