Título: CLINICOPATHOLOGICAL AND SONOGRAPHIC DIAGNOSIS OF LATE-ONSET FOREIGN BODY REACTION TO SILICONE LIP FILLER: A CASE REPORT

Resumo: A 74-year-old man presented with multiple normochromic, flaccid, and painless nodules on the upper and lower lip mucosa, causing a double-lip-like deformity. The lesions emerged three years prior. The patient disclosed a history of cosmetic lip fillers of unknown composi-tion eight years before the appointment. Ultrasound revealed hyperechoic submucosal de-posits with snowstorm artifacts extending into the orbicularis oris muscle, suggesting sili-cone oil. An incisional biopsy showed connective tissue with optically clear vacuoles con-sistent with exogenous material, macrophage infiltration, and significant vascularization, establishing the diagnosis of a foreign body reaction. A cosmetic surgical excision of the remaining nodules was performed one month later, with histopathology confirming a foreign body granuloma. At the 3-week postoperative follow-up, the patient demonstrated proper healing and excellent aesthetic results.
