Resumo: A 41-year-old homeless man with active tuberculosis and HIV infection, with no adherence to antiretroviral therapy, presented with an asymptomatic lesion on the hard palate with a six-month history of progression. Clinical examination revealed a solitary, purplish, sessile nodule with a rough surface, measuring approximately 2 cm. An incisional biopsy was performed, and the diagnostic hypothesis was Kaposi sarcoma (KS). Histopathological analysis revealed a malignant neoplasm of vascular origin, characterized by the proliferation of spindle-shaped endothelial cells within the connective tissue, formation of irregular vascular slits, and extravasated erythrocytes. Immunohistochemical analysis showed positivity for CD31, CD34, and HHV-8, confirming the diagnosis of KS. The patient was referred for medical follow-up, including the reinitiation of antiretroviral therapy and management of tuberculosis. This case highlights the importance of early diagnosis of oral lesions associated with HIV, particularly in vulnerable populations, contributing to improved prognosis and systemic management.
