Título: DOES PORPHYROMONAS GINGIVALIS UPREGULATE PD-L1 EXPRESSION IN CANCER CELLS? A SYSTEMATIC REVIEW OF IN VITRO STUDIES

Resumo: Objective: To evaluate the effect of P. gingivalis exposure on PD-L1 expression in cancer cells based on evidence from in vitro experimental studies. Methods: A systematic review of in vitro experimental studies was conducted following PRISMA 2020 guidelines. Human cancer cell lines from different anatomical sites (oral cavity, esophagus, colon and prostate) were included. PD-L1 expression was assessed after exposure to P. gingivalis or its components (live bacteria, membrane fractions, peptidoglycan, lipopolysaccharide and outer membrane vesicles) using qPCR, Western blot and flow cytometry. Risk of bias was evaluated using the QUIN tool. Results: Six studies met the inclusion criteria. All demonstrated increased PD-L1 expression following exposure to P. gingivalis or its components. The magnitude of upregulation ranged from approximately 2- to 11-fold and was consistently observed across different cancer cell types and experimental conditions, including variations in bacterial form, concentration and exposure protocols. Conclusion: P. gingivalis consistently upregulates PD-L1 expression in cancer cells in vitro, suggesting a potential role in tumor immune evasion. Further studies are required to determine its translational relevance and impact on immunotherapeutic strategies.
