Título: PYOGENIC GRANULOMA WITH FOCI OF MASSON'S TUMOR: A RARE HISTOPATHOLOGICAL ASSOCIATION

Resumo: A 34-year-old female patient presented with a sessile, erythematous nodule on the upper lip, measuring approximately 4 mm in diameter. The lesion had appeared two months earlier and was asymptomatic, firm on palpation, and smooth-surfaced. The clinical hypothesis was pyogenic granuloma, and an excisional biopsy was performed. Microscopic examination revealed a highly vascularized, non-neoplastic proliferative lesion composed of newly formed blood vessels arranged in a lobular pattern with young fibroblasts. A focal area showed blood vessels of varying diameters lined by regular endothelium, proliferating in a papillary pattern toward the lumen of a dilated vessel, consistent with papillary endothelial hyperplasia (Masson's tumor). The Masson component represents a secondary reactive proliferation within the vascular lesion, often arising from organized thrombi. The main concern is to differentiate it from malignant vascular tumors, such as angiosarcoma. Treatment consisted of complete surgical excision, and the patient remains under follow-up, with low risk of recurrence.
