Resumo: This study investigated the relationship between bone mineral density and bone invasion in patients with oral cavity squamous cell carcinoma. A retrospective, single-center cohort study was conducted with 120 patients diagnosed in 2023. Radiomorphometric indices, including the mandibular cortical index, fractal dimension, mandibular index, and W index, were analyzed and correlated with demographic data, risk habits, tumor characteristics, and clinical outcomes. Bone invasion was observed in 17.5% of cases, predominantly affecting the mandible (76.2%). Patients with bone invasion were significantly older (p = 0.017) and had a higher prevalence of smoking (p = 0.043) and alcohol consumption (p < 0.05). The mandibular cortical index (C2/C3) was more frequent in patients with invasion (p = 0.031), while fractal dimension was significantly lower (p = 0.004), indicating reduced bone quality. Logistic regression analysis demonstrated that decreased BMD increased the risk of bone invasion by up to threefold (OR = 3.01; p = 0.039), with a higher risk observed in women (OR = 3.42; p = 0.034). Additionally, patients with bone invasion showed significantly shorter survival (5.3 vs. 13 months; p = 0.021). These findings suggest that radiomorphometric indices from panoramic radiographs provide valuable prognostic information, supporting risk stratification and treatment planning.
