Resumo: Introduction: Adenoid ameloblastoma is a rare odontogenic tumor, recently recognized in the 2022 WHO classification. It is characterized by cribriform architecture, duct-like structures, dysplastic dentinoid deposits, and absence of BRAF mutations.

Objective: To present three clinical cases of adenoid ameloblastoma, including histopathological and immunohistochemical evaluation.

Methods: Three female patients between the fourth and fifth decades of life were analyzed. They presented with radiolucent and radiomixed mandibular lesions with sclerotic borders, all asymptomatic. Incisional and excisional biopsies were performed. Histopathological studies revealed odontogenic epithelium with cribriform, follicular, and plexiform patterns, stellate reticulum-like areas, ductal cells, and dysplastic dentinoid formation. Immunohistochemical analysis using the BRAF marker was negative in all cases.

Conclusion: These findings highlight the distinct clinicopathological features of adenoid ameloblastoma and support its classification as a separate entity. The absence of BRAF mutations suggests a molecular pathogenesis linked to β-catenin alterations, emphasizing the importance of accurate diagnosis and molecular characterization.
