Título: PARANEOPLASTIC VESICULOBULLOUS LESIONS ASSOCIATED WITH TYPE AB THYMOMA: A CASE REPORT

Resumo: A 59-year-old male presented with severe oral burning interfering with daily activities. His medical history included controlled pemphigus foliaceus and refractory oral lichen planus. Intraoral examination revealed widespread ulcerations and white striae on the buccal mucosa, tongue, and floor of the mouth. A new biopsy demonstrated a subepithelial bulla. Systemic investigation via chest radiography and gallium-68 imaging identified a mediastinal mass near the aorta. The tumor was biopsied and diagnosed as type AB thymoma, followed by robotic-assisted surgical resection. The final diagnosis was established as paraneoplastic vesiculobullous lesions associated with thymoma. Management involved tapering immunosuppressive therapy (prednisone and azathioprine) and implementing a Lifestyle Medicine intervention based on the "8 Pillars of Health." At the one-year follow-up, the patient remained asymptomatic and lesion-free, showing significant improvements in quality of life.
