Título: SAPHO SYNDROME WITH MANDIBULAR INVOLVEMENT: A MULTIDISCIPLINARY CHALLENGE

Resumo: We present the case of a 20-year-old female patient with a one-year history of chronic pain, swelling, and functional limitation in the posterior mandibular body and ramus following orthodontic treatment. Notably, the patient was being treated for acne, though the skin lesions were initially managed without considering a correlation to her musculoskeletal symptoms. Clinical evaluation, CBCT imaging, and biopsy confirmed chronic non-bacterial osteomyelitis, with radiological evidence of characteristic osteolysis and hyperostosis. The final diagnosis was SAPHO syndrome with specific mandibular and temporomandibular joint involvement. A multidisciplinary treatment plan was initiated, including a tapering Prednisone regimen, methotrexate, and folic acid. Following the start of therapy, the patient showed significant symptomatic improvement, with a marked reduction in pain and swelling. She remains under close rheumatological surveillance to monitor functional recovery and prevent further pathological bone remodeling, highlighting the importance of early multidisciplinary diagnosis.
