Resumo: Several radiolucent lesions can affect the anterior region of the mandible and exhibit aggressive behavior, such as odontogenic keratocyst, conventional ameloblastoma, central giant cell lesion, and odontogenic myxoma. In this case, a 24-year-old female patient, melanoderma, asymptomatic, sought the Oral Surgery service for evaluation of a lesion in the anterior mandibular region, identified on a routine panoramic radiograph. Cone-beam computed tomography revealed that the lesion involved multiple dental elements and caused thinning and destruction of the bony cortices. Incisional biopsy was performed in an outpatient setting under local anesthesia, and the specimen was sent to the oral pathology laboratory. The histopathological diagnosis was periapical cemento-osseous dysplasia. This condition is a benign fibro-osseous lesion that does not require treatment and is characterized by the replacement of bone with fibrous tissue. The patient did not present complications and remains under follow-up by the oral surgery team.
