Resumo: Primary intraosseous carcinoma (PIOC) is a rare malignancy of the jawbones, but its development arising from a pre-existing odontogenic cyst is particularly uncommon. This case report describes a 37-year-old female with a rapidly enlarging palatal swelling and a history of prior cystic lesion enucleation at the same site. Clinical findings included palatal and buccal swelling, tooth mobility, and lymphadenopathy. Radiographic imaging revealed alveolar bone destruction extending into the maxillary sinus. Treatment involved a left partial maxillectomy and bilateral neck dissection. Histopathological and immunohistochemical analysis (AE1/AE3, CK19, and high Ki-67) confirmed PIOC arising from an odontogenic keratocyst. This case emphasizes the importance of thorough follow-up for seemingly benign odontogenic cysts, as they may undergo a rare malignant transformation into aggressive squamous cell carcinomas.
