Resumo: Odontogenic myxoma is a rare benign mesenchymal neoplasm. A 9-year-old male patient presented with a two-week history of swelling in the anterior mandible, involving the region of teeth 74 to 84. Clinical examination revealed a 4 cm lesion affecting both buccal and lingual cortices, with firm consistency, smooth surface, regular contours, and normal-colored mucosa externally and intraorally. Computed tomography revealed a hypodense, multilocular expansile lesion with cortical thinning and internal septations, involving adjacent tooth structures. Incisional biopsy revealed fibrous tissue with gelatinous areas, and histopathological analysis demonstrated myxoid tissue with spindle-shaped and stellate mesenchymal cells in an abundant extracellular matrix, confirming odontogenic myxoma. The patient underwent surgical treatment and remains under follow-up due to the risk of recurrence.
