Resumo: Syphilis is a sexually transmitted infection caused by the bacterium Treponema pallidum, which progresses through different stages and may resemble other diseases. Case report: 29-year-old male patient sought dental care complaining of a tongue ulcer that had been present for three weeks and which, following an accidental bite, had begun to bleed and cause pain. The clinical examination revealed a single ulcer with hardened edges, located on the side of the tongue, without lymphadenopathy. Serological tests for syphilis were requested; the VDRL and FTA-ABS tests were nonreactive. An incisional biopsy was performed, with the histopathological report showing a lymphoplasmacytic inflammatory infiltrate; special silver staining confirmed the presence of spirochetes and the diagnosis of syphilis. The patient was referred to a doctor and treated with benzathine penicillin, with resolution of the disease. This case highlights the importance of the differential diagnosis of oral ulcers, their diagnostic significance, and clinical implications.
