Resumo: Oral squamous cell carcinoma (OSCC) in young patients without traditional risk factors poses diagnostic and prognostic challenges. Two male patients (21 and 35 years), non-smokers and non-drinkers, presented with OSCC of the lateral tongue and underwent identical initial management with partial glossectomy and supraomohyoid neck dissection, with tumor-free margins and p16 negativity. Despite similar clinicopathologic features, outcomes diverged. The younger patient developed early recurrence within one month, followed by pulmonary metastasis and aggressive progression, requiring chemotherapy, radiotherapy, and immunotherapy (pembrolizumab; PD-L1 CPS=25). The second patient showed favorable evolution without recurrence. Flow cytometry revealed a high proportion of aneuploid nuclei in the aggressive tumor, whereas the favorable case exhibited a low-risk DNA profile. These findings suggest that DNA aneuploidy may indicate aggressive behavior and support early risk stratification. Additional proteomic analysis using the GigaTIME platform is currently underway.
