Resumo: Objective:
To identify the sociodemographic, clinicopathological, and treatment-related factors influencing local recurrence (LR) in oral squamous cell carcinoma (OSCC).
Study design:
A retrospective cohort study of 260 patients with surgically treated primary OSCC from 2005 to 2023 was conducted. Sociodemographic, clinicopathological, and treatment-related data were analyzed. Univariate logistic regression identified variables associated with LR (p < 0.25), which were entered into a multivariable logistic regression model using backward selection.
Results:
LR was observed in 13.5% (35/260) of cases. Tumour dimension ≥28 mm (OR = 2.38, p = 0.039) and age ≥61 years (OR = 2.60, p = 0.022) were significantly associated with increased odds of developing LR. Post-adjuvant therapy was also significantly associated with LR, with radiotherapy (OR = 2.83, p = 0.025) and radiochemotherapy (OR = 7.98, p < 0.001) demonstrating higher odds. This likely reflects underlying disease severity. Nodal involvement was significant overall (p = 0.027), although individual subgroup comparisons were not statistically significant.
Conclusion:
Tumour size and age emerged as independent predictor of LR in OSCC. The association with post-adjuvant therapy likely reflects underlying disease severity rather than a direct treatment effect. These findings highlight the importance of risk stratification and careful postoperative surveillance in high-risk patients.
