Resumo: Benign mucous membrane pemphigoid is a rare autoimmune disorder characterized by vesicle formation and subsequent painful ulcerations in oral mucosa. A 47 year old female presented with gingival pain and halitosis, with lesions appearing one year earlier following a major stress episode. Clinical examination revealed desquamative gingivitis and the presence of a single vesicule. An incisional biopsy demonstrated a subepithelial cleft and moderate diffuse chronic inflammatory infiltrate in the lamina propria, consistent with benign mucous membrane pemphigoid. The patient was treated with topical corticosteroids and remains under followup. This case highlights the importance of recognizing benign mucous membrane pemphigoid as a differential diagnosis in patients presenting with desquamative gingivitis. The oral manifestations, often mistaken for chronic inflammatory gingival conditions, require careful clinical and histopathological evaluation to establish an accurate diagnosis. Early identification is essential, as mucosal involvement may extend to the eyes and other sites, making an interdisciplinary approach indispensable.
