Resumo: Medication-related osteonecrosis of the jaw (MRONJ) is a complication associated with antiresorptive and antiangiogenic drugs, characterized by exposed bone in the oral cavity, spontaneously or after local trauma. A 76-year-old hypertensive female with a history of antiresorptive use presented with mandibular pain three years after implant placement. Clinical examination revealed edema, submental fistula with purulent drainage, and bone exposure in the mandibular region. Tomographic imaging showed areas of necrosis and sequestration. The patient underwent multiple interventions, including antibiotic therapy, surgical debridement, implant removal, leucocyte- and platelet-rich fibrin membrane, antimicrobial photodynamic therapy (aPDT) and ozone therapy, due to recurrent infection, fistulas and bone exposure. Clinical and imaging findings, and histopathological analysis of bone fragments confirmed the diagnosis of MRONJ. After six years, complete resolution was achieved with a multimodal approach including sequestrectomy, aPDT, and ozone therapy. This case highlights the challenges in MRONJ management and the need for combined therapeutic strategies.
