Título: ORAL NON-TUBERCULOUS MYCOBACTERIOSIS IN A RHEUMATOID ARTHRITIS PATIENT

Resumo: A 63-year-old female patient, under treatment with methotrexate and adalimumab for rheumatoid arthritis, presented with a two-month history of pain and facial edema. Physical examination revealed locoregional lymphadenopathy, and an intraoral granulomatous ulcerated lesion in the right vestibular fornix. Incisional biopsy was performed with the differential diagnosis of infection or carcinoma. Histopathological analysis demonstrated a chronic inflammatory infiltrate with granuloma formation, multinucleated giant cells, epithelioid macrophages, and foci of necrosis. Histochemical staining was negative for fungal organisms and Acid-Fast Bacilli. Given these findings, the patient was referred to an infectious diseases hospital and a pulmonary lesion was detected. Molecular investigation identified Mycolicibacterium phocaicum/mucogenicum, establishing the final diagnosis of nontuberculous mycobacteriosis. A triple-drug regimen of rifampicin, moxifloxacin, and clarithromycin was prescribed for one year. The lesion showed complete regression at 10-month follow-up. This case highlights the increased risk of deep infectious lesions in patients receiving immunosuppressive therapy for rheumatoid arthritis.
