Resumo: ​Leukoderma female, 65 years old, presented to an emergency dental service at the School of Dentistry of the University of São Paulo with three months of severe oral mucosal pain, dysphagia, dyspnea, and multiple oral and nasal ulcerations. She had been previously evaluated and treated with Dispropan®️, with an initial suspicion of Stevens-Johnson syndrome. Laboratory tests and nasopharyngoscopy were requested, and prednisone 80 mg/day was started with tapering to 20 mg/day. Endoscopy showed no oropharyngeal lesions and a mild crusted nasal lesion, suggesting drug-associated erythema multiforme
​After partial clinical improvement, an incisional biopsy revealed ulceration with herpesvirus cytopathic effects and interface mucositis, consistent with a lichenoid process. These findings suggest a mucosal reaction pattern possibly triggered by viral infection rather than a single defined disease. The case highlights the complexity of multifactorial oral lesions and the importance of structured clinical reasoning and follow-up.
