Título: EOSINOPHILIC ULCER: A CASE REPORT IN AN ELDERLY PATIENT WITH COGNITIVE IMPAIRMENT

Resumo: An 81-year-old female patient with Alzheimer's disease presented with a 17-day history of an ulcer on the right lateral border of the tongue. Clinical examination revealed a lesion with a white-yellowish base, slightly elevated margins, and firm consistency on palpation, raising suspicion of malignancy. Due to cognitive impairment, the patient was unable to report local trauma; however, a fractured restoration on tooth 45 suggested chronic irritation. Exfoliative cytology was performed to rule out malignancy, with negative results. The caregiver was instructed, and all possible local traumatic factors were removed. Photobiomodulation with low-level laser therapy (LLLT) was initiated, but no clinical improvement was observed after five sessions. An incisional biopsy was then performed. Histopathological analysis showed fibrous connective tissue with intense inflammatory infiltrate, including eosinophils. Following biopsy and continued LLLT, the lesion healed rapidly, and the final diagnosis of eosinophilic ulcer was reached. The patient remained under follow-up without further complaints.
