Resumo: Actinic cheilitis is a potentially malignant disorder identified on the vermilion of the lower lip, associated with chronic exposure to UV radiation. Early diagnosis and appropriate management are essential to prevent progression to squamous cell carcinoma, one of the deadliest non-melanoma skin cancers. Several different techniques have been proposed for the management of actinic cheilitis, including tissue ablation using a high-power diode laser. This technique stands out as a minimally invasive option, capable of removing affected tissue with minimal bleeding and promoting optimal tissue healing. This report describes two cases of actinic cheilitis in middle-aged patients with a history of occupational sun exposure, with histopathological findings of severe epithelial dysplasia. The proposed treatment was ablation with a high-power diode laser. The patients demonstrated satisfactory clinical healing, with no evidence of recurrence, reinforcing the potential of this therapy as an effective and more conservative treatment compared with vermilionectomy.
