Título: Salivary Microcosm Biofilm Formation in Hookah Users, Conventional Cigarette Smokers, and Non-Smokers

Resumo: The present study aimed to evaluate the capacity of saliva from hookah and conventional cigarette users to form microcosm biofilms, and to compare these findings with those from non-smokers. Whole saliva samples were collected and allocated into three groups: frequent, exclusive hookah users; frequent, exclusive conventional cigarette users; and a control group of non-smokers. Biofilms derived from saliva samples were evaluated for total biomass, metabolic activity, and microbial load. Biomass levels were significantly higher in hookah (5.15) and conventional cigarette users (6.17) compared to controls (2.54; p < 0.001). Similarly, metabolic activity was higher in the hookah (1.50) and cigarette (1.68) groups than in controls (1.10). Non-selective microbial counts were higher in hookah users, differing significantly only from non-smokers, while conventional cigarette smokers exhibited intermediate values without statistical significance. No significant differences were observed among the groups in CFU/mL counts of Streptococcus mutans, Lactobacillus spp., and Candida spp. The observed increase in biomass formation and metabolic activity suggests that tobacco exposure disrupts the balance of the oral ecosystem, favoring microbial dysbiosis. This process may promote the growth of anaerobic and periodontopathogenic microorganisms not detected in the present culture-based analysis.
