Resumo: A 32-year-old white woman presented to routine dental appointment with no complaints. Her medical history was uneventful. Upon physical examination, a sessile firm nodule, smooth-surfaced, normochromic with a purple shade in the center, with 1.5 centimeters was detected in the lower gingiva between second and third molars. Due to first molar absence, contact point between molars was unsatisfactory. A computed tomography revealed mineralized tissue within the nodule image, leading to a clinical hypothesis of peripheral ossifying fibroma. Excisional biopsy was performed and histopathology revealed stratified squamous epithelium and underlying connective tissue with multiple giant cells; areas of hemorrhage and hemosiderin and deeper mineralized tissue of varying degrees of ossification and sizes was observed. The final diagnosis was peripheral giant cell lesion. The dentist asked for serum levels of parathormones, and they were within the normal range. The patient is under follow-up for one year and no recurrence was detected.
