Resumo: This study reports the clinical and histopathological progression of an oral leukoplakia over a ten-year period, culminating in squamous cell carcinoma. A 52-year-old female patient presented with a persistent white lesion on the right lateral border of the tongue, associated with pain and burning sensation. Her medical history included type 2 diabetes mellitus with poor glycemic control and a previous history of smoking. Initial clinical diagnosis was oral leukoplakia, and serial biopsies were performed throughout follow-up. Histopathological findings demonstrated a gradual progression from mild dysplasia (2016-2017) to moderate dysplasia (2018), advancing to severe dysplasia (2023). Delayed surgical intervention due to limited healthcare access and metabolic instability resulted in lesion excision only in 2025, when a moderately differentiated squamous cell carcinoma was confirmed. This case highlights the critical importance of long-term surveillance and timely intervention in potentially malignant oral disorders, particularly in patients with systemic comorbidities and established risk factors.
