Resumo: Objective: To investigate associations between smoking-related variables, patients age, and oral mucositis (OM) severity in patients undergoing head and neck radiotherapy (RT). Design: This prospective, longitudinal clinical trial involved 58 patients. OM was graded via the WHO scale at five time points (T1-T5 - RT week and the development of the OM). Smoking status, intensity, duration, pack-years, and age group (≤50 vs. >50 years) were analyzed using Spearman's correlation, repeated-measures ANOVA, Fisher's exact test, and multiple linear regression. Results: Cumulative smoking burden, including pack-years and duration, showed no significant correlation with OM severity at any interval (p > 0.05). Severity did not vary significantly by age group (ANOVA, p > 0.05), and Fisher's exact test confirmed no difference in OM onset time between age groups (p = 0.113). However, multiple linear regression for T4 identified active smoking as a significant independent predictor of greater OM severity (ꞵ = 0.879; p = 0.036; R² = 0.596). Conclusion: OM severity was unrelated to cumulative smoking history or age but was significantly influenced by active smoking during RT. These findings suggest tobacco use has a direct effect on tissue radiation response, emphasizing the critical importance of smoking cessation during treatment.
