Resumo: This report describes a diagnostically challenging case of recurrent oral erythema multiforme with an extensive investigative and therapeutic course. A 44-year-old female presented with a two-month history of painful oral lesions unresponsive to prior treatments. Clinical features suggested erythema multiforme associated with herpes simplex infection, without involvement of other mucosal sites. The patient experienced recurrent episodes characterized by irregular, ulcerated white-red lesions affecting the buccal mucosa, soft palate, tongue, and floor of the mouth, preceded by systemic prodromes. Serological tests were positive for herpes simplex virus types 1 and 2. Initial management included antiviral therapy, corticosteroids, and discontinuation of suspected medications. Multiple biopsies yielded conflicting diagnoses, including mucous membrane pemphigoid, lichen planus, and erythema multiforme. Lack of response to methotrexate prompted further investigation. Direct immunofluorescence and histopathological review confirmed erythema multiforme, with complete clinical remission achieved using dapsone, highlighting its role in refractory recurrent cases.
