Título: MALIGNANT ORAL LESION MIMICKING OSTEOMYELITIS: PHOTOBIOMODULATION MANAGEMENT - A CASE REPORT

Resumo: A male patient with controlled diabetes mellitus, hypertension, and a cardiac pacemaker presented with a painful lesion in the lower right molar region following trauma associated with a removable partial denture. The condition progressed with feeding difficulty and tooth mobility, leading to extraction under an initial diagnosis of osteomyelitis. Due to lesion persistence, an incisional biopsy was performed. Histopathological analysis confirmed a malignant neoplasm, and immunohistochemistry established the diagnosis of Diffuse Large B-Cell Lymphoma. The patient underwent combined therapy consisting of three cycles of chemotherapy and 15 sessions of radiotherapy and subsequently developed oral mucositis, radiodermatitis, and hyposalivation. Intra- and extraoral photobiomodulation using the Gemini Evo device was performed for symptom control and tissue repair. PET-CT demonstrated no evidence of active disease, with a Deauville score of 1, indicating complete metabolic response. The patient exhibited pain reduction, improvement in oral conditions, and functional recovery.
