Resumo: Metastases to the oral and maxillofacial region account for approximately 1% of all malignant lesions in this area. A 74-year-old woman presented with a seven-month history of progressive left mandibular enlargement. Her medical history included previous uterine and breast malignancies and a recent diagnosis of colorectal adenocarcinoma with liver metastases. Clinical examination revealed pain, facial asymmetry, local paresthesia, persistent tinnitus, and ipsilateral lymphadenopathy. Cone beam computed tomography showed a mixed osteolytic lesion on the left side of the mandible, with periosteal and trabecular alterations, raising suspicion of metastatic disease. An incisional biopsy was performed, and histopathological analysis revealed a malignant epithelial neoplasm with pleomorphic hyperchromatic cells, focal ductal architecture, and bone destruction. Immunohistochemical reactions showed CK20+ and CK7-, supporting gastrointestinal origin and confirming the diagnosis. PET-CT revealed disseminated hypermetabolic lesions in soft tissues and the skeleton. She was referred for treatment and remains under medical and dental follow-up.
