Resumo: A 13-year-old male patient with a previous diagnosis of chronic granulomatous disease, an X-linked primary immunodeficiency, was referred to an Oral Medicine service for evaluation due to recurrent chronic lip swelling for approximately three years. Extra and intraoral examination revealed diffuse edema of the upper and lower lips, associated with a cobblestone appearance of the bilateral buccal mucosa. The diagnostic hypothesis was orofacial granulomatosis associated with the underlying disease, based on clinical history, physical examination, and prior genetic diagnosis. Incisional biopsy revealed squamous mucosa with mild chronic inflammation, predominantly perivascular and juxtaepithelial, minimal fibrosis, and an epithelioid microgranuloma in the corium.The patient has restriction of systemic corticosteroids. Initial treatment with clobetasol propionate topical gel (0.05%, three times/day, thirty days) showed no significant response, while topical tacrolimus (0,03%, twice a day, forty days) led to slight improvement. The patient remains under multidisciplinary follow-up, with planned intralesional corticosteroid therapy during exacerbations.
