Título: MULTIPLE PAINLESS ULCERATIONS OF THE ORAL CAVITY

Resumo: A 41-year-old man arrived with painless ulcerations on the palate and labial mucosa plaques for three weeks' duration, alongside a two-year progression of pruritic skin lesions and erythroderma. An intraoral examination revealed an extensive 6 cm erythematous, ulcerated lesion on the bilateral hard and soft palate. The diagnostic procedures involved incisional biopsies and immunohistochemistry, which revealed a clonal population of helper T-cells (CD3+, CD4+) with an aberrant loss of the pan T-cell marker CD5 and CD8. The clinical and histopathological results confirmed CTCL-MF, however the diagnosis of SS was established after correlation with additional clinical, hematological, and immunophenotypic criteria. The management included total skin electron beam therapy (TSEBT) and systemic chemotherapy with gemcitabine. Following a systemic relapse and largecell transformation on a lymph node, treatment was transferred to the CHOEP protocol, with allogeneic hematopoietic stem cell transplantation for curative intent with a specific donor because of the significant limitation.
