Resumo: Neurovascular hamartoma (NVH) is an extremely rare pathology characterized by the disorganized proliferation of neural and vascular components. Due to its histological components and anatomical location, it presents a diagnostic and therapeutic challenge. A 57-year-old man attended a routine dental examination presenting a asymptomatic swelling on the lower lip since childhood and multiple nodular-like lesions on the dorsal tongue. The initial clinical hypothesis was schwannoma involving the labial region and lymphangioma affecting the tongue. Incisional biopsies were performed on the lesions of the lip and tongue and histopathological analysis revealed a poorly circumscribed proliferation of mature nerve bundles in close proximity with prominent blood vessels, within a loose connective tissue matrix, consistent with a NVH diagnosis. The management of this case consisted of surgical excision. This case highlights a rare extra-lingual presentation and emphasizes the importance of histopathological evaluation for an accurate diagnosis.
