Título: ANTIMICROBIAL PHOTODYNAMIC THERAPY TO REDUCE MRONJ RISK AFTER TOOTH EXTRACTION: A RANDOMIZED PILOT STUDY

Resumo: Objective: To evaluate the feasibility of antimicrobial photodynamic therapy (aPDT) in mucosal repair after dental extractions in cancer patients at risk of medication-related osteonecrosis of the jaw (MRONJ). Materials and methods: Randomized pilot clinical trial carried out in a referral oncology hospital with 18 patients using MRONJ. Participants were randomly assigned to two groups: aPDT (tooth extraction + aPDT + photobiomodulation) and ATB (tooth extraction + systemic oral antibiotic therapy + photobiomodulation). The clinical outcomes evaluated were analgesic use, infection, mucosal healing and bone exposure. Results: Each group underwent a total of 18 tooth extractions. Metastatic breast cancer was the most frequent diagnosis and zoledronic acid was the most commonly used antiresorptive. Carious lesions were the main indication for tooth extraction. No cases of postoperative infection were observed and no analgesics were used after the 14th day in either group. Alveolar mucosa healing and bone exposure evolved similarly between groups (p ≥ 0.317). Regarding bone exposure, comparisons between groups revealed no statistically significant differences, with p = 1.000. Conclusion: aPDT has demonstrated similar efficacy to systemic antibiotic therapy in reducing the risk of MRONJ, proving to be a promising and safe alternative.
