Resumo: Solitary Fibrous Tumor (SFT) is an uncommon mesenchymal neoplasm of uncertain etiology. Although it primarily affects the pleura, it has also been reported in extrapleural sites, where it generally behaves benignly. In the oral cavity, it predominantly affects the buccal mucosa and tongue of women between the 4th and 5th decades of life. An 81-year-old woman presented with a firm, color-matched mass on the lower lip mucosa that had been present for one year, with a clinical diagnostic hypothesis of lipoma. An excisional biopsy was performed, and histopathological examination revealed a biphasic pattern of spindle cell proliferation surrounding blood vessels with irregular branching. Alternating hypocellular and hypercellular areas were observed, with a predominance of dense fibrous connective tissue and no signs of malignancy. Immunohistochemical analysis was positive for STAT6, CD34 and Bcl-2, supporting the diagnosis of SFT. The patient has been under follow-up for 10 months with no recurrence.
