Resumo: Giant cell fibroma is a benign fibrous tumor unrelated to local irritative factors, commonly found on the gingiva of young women, and has an excellent prognosis. A 48-year-old woman presented with the chief complaint of an aesthetic concern due to an anterior gingival overgrowth with a five-year duration. Physical examination revealed a normochromic, asymptomatic, sessile nodule with a smooth surface, located between teeth 43 and 44, measuring 2.0x1.2 cm. The differential diagnoses included peripheral ossifying fibroma and inflammatory fibrous hyperplasia. An excisional biopsy was performed, and histopathological analysis revealed numerous multinucleated stellate fibroblasts in the subepithelial region within a loosely arranged fibrous stroma. Immunohistochemistry demonstrated strong vimentin expression in these cells, whereas CD163, α-SMA and S-100 were negative. The final diagnosis was giant cell fibroma. The patient was informed about her condition, and a six-month follow-up showed no signs of recurrence. This work was supported by FAPESP (Process: 2025/14452-8).
