Título: FLORID CEMENTO-OSSEOUS DYSPLASIA ASSOCIATED WITH CHRONIC OSTEOMYELITIS TREATED WITH THE PENTOCLO PROTOCOL: A CASE REPORT

Resumo: A 46-year-old black female patient was referred to the clinic with suspected florid cemento-osseous dysplasia (FCOD), a fibro-osseous condition that may be complicated by secondary infection. Panoramic radiography and CBCT revealed multiple intramedullary hyperdense masses, with hypodense halos in the mandibular periapical region, associated with cortical expansion and thinning. In the region of tooth 36, a periradicular hypodense lesion, bone loss and furcation involvement were observed, suggesting chronic osteomyelitis. Based on clinical and radiographic findings, the diagnosis of FCOD associated with chronic osteomyelitis was established. Treatment included systemic antibiotic therapy targeting anaerobic microbiota, antimicrobial photodynamic therapy for local decontamination and the PENTOCLO protocol to improve perfusion, modulate inflammation and promote bone remodeling. After 2 months, the patient showed good clinical evolution, with bone repair, no systemic adverse effects and remains under periodic clinical and imaging follow-up, due to the risk of recurrence.
