Resumo: An 80-year-old female patient, former smoker, presented with a sessile, well-circumscribed lesion on the right soft palate, exhibiting a smooth surface and bluish discoloration. The lesion was asymptomatic and firm on palpation. During intraoral physical examination, an exploratory aspiration was performed, yielding a negative result, suggesting a solid lesion. The initial diagnostic hypothesis was a malignant salivary gland neoplasm. Incisional biopsy revealed fibrous connective tissue with a diffuse mononuclear inflammatory infiltrate, predominantly lymphocytic with plasma cells, suggestive of a lymphoproliferative disorder. Immunohistochemical analysis demonstrated atypical B-cell proliferation, consistent with small B-cell lymphoma. The patient was referred to a specialized oncology reference center. This case underscores the importance of clinicopathological correlation, highlights the role of immunohistochemistry, and emphasizes the need to include extranodal lymphoma in the differential diagnosis of oral lesions that clinically mimic salivary gland malignancies.
